    Mechanisms of Advanced NAFLD Disparities in Hispanics: A Multi-level Analysis

    Description: Non-alcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease in modern society. NAFLD is predicted to become the leading indication for liver transplantation and is a risk factor for the development of hepatocellular carcinoma that can develop even without cirrhosis. In a large Hispanic and non- Hispanic White population with unequivocally staged levels of fibrosis, we will combine state-of-the-art approaches (e.g., lifestyle and environmental factors, unbiased genetic variation analyses, single-cell transcriptomic analyses of peripheral innate immune cells resulting in identification of diagnostic protein biomarkers, and structural integrated analyses) to gain novel mechanistic insights into NAFLD disparities in Hispanics and disease progression to fibrosis which is the single most important predictor of outcome.

NAFLD is predicted to become the leading indication for liver transplantation and is a risk factor for the development of hepatocellular carcinoma that can develop even without cirrhosis. The lack of evidence-based, FDA-approved treatment options for NASH underscores the need for further research into understanding disease pathogenesis and identifying potential targets. Here, based on compelling preliminary and recently published data from our group, we propose to comprehensively study how the innate immune response mediates inflammation and fibrosis in mice and humans with NAFLD, providing novel insights into potential therapeutic targets.
    Global liver specific gene expression changes induced by PFAS and risk of liver cancer

    Description: We will combine two state-of-the art technologies (3D liver culture and scRNA- Sequencing) to establish a robust physiologically relevant model to test not only the effects of PFAS chemicals on human liver, but this model will also prove useful to screen other damaging agents as well as potential therapeutics. We will generate a high-resolution data set on human liver responses to important legacy and replacement PFAS chemicals. The pilot project will provide us with insights and preliminary data into the relationship between biological pathways in the liver and environmental pollutants and will form the basis for an R01 application. In the R01 submission, we plan to expand the analysis of human study to include NAFLD and cirrhosis cases in the MEC and samples from other cohorts.
